Weapon and armor upgrades can be received as a reward for completing quests and war table operations, looted from enemies, purchased from select merchants, found in chests and containers, or crafted at any crafting table. In either case, the type of potential upgrade slot solely depends on the type of equipment acquired or created.
Similarly, crafted weapons and armors are created with predetermined upgrade slots affixed to the equipment.
In non-crafted weapons and armors, upgrade slots are pre-existing, new slots cannot be added, only modified.
Most weapons and armors acquired in Dragon Age: Inquisition have upgrade slots that can be modified.
